Choosing a format
You may download the forms in either of two formats:
Adobe Portable Document Format (PDF). If you download the PDF version of a form, you will have to print it out first, then fill it in by whatever means you find most convenient. To view and print PDF files, you will need a PDF reader, such as the Adobe Acrobat® reader. If you do not have the Acrobat reader, it is available free for a range of common operating systems from the Adobe web site. (Alternatively, if you are an X Windows user on UNIX, VMS, or OS/2, you might like Derek Noonburg’s free Xpdf PDF viewer.)
Microsoft Word (MS Word). If you have the Microsoft Word word processing program, you might prefer to download the MS Word version of a form. This will allow you to open and fill in the form using your word processor, then print it out when complete.
The forms are typically two or three pages in length, and none of the files is greater than about 30 Kbytes in size. If you are unable to download the form you need for any reason, please contact the Tribunal.
Preparing and submitting forms
Please note:
- Although the MS Word versions allow for free editing in Word, you must attempt as far as possible to maintain the style and layout of the form.
- However you choose to complete your application, you must print and sign the forms, and post them to the Tribunal’s offices. The Tribunal cannot accept applications submitted by e-mail.
